Notice of Availability: IRS Form 1095-C for the 2025 Tax Year

In accordance with the Internal Revenue Code and the Affordable Care Act (ACA), Waring Oil Company, LLC. is providing notice that your Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) for the 2025 calendar year is available upon request.

What is Form 1095-C? This form provides information about the health insurance coverage offered to you by your employer. While you are not required to submit this form with your tax return, you may need it for your records or to verify your coverage status.

How to Request a Copy: You have the right to receive a paper copy of your Form 1095-C free of charge. To request your copy, please use any of the following methods:
